*Job Title: Personal Support Worker (PSW) – Palliative / ALS Care*
*Location:* Burlington, ON
*Patient Condition & Background*
* Diagnosed with ALS in May 2024
* Significant physical decline; was ambulatory until May 2025 when frequent falls began
* Currently able to stand and weight bear for very short periods only
* Becomes easily lightheaded and stressed with exertion
* Spends most of the day in a lift chair
* Increasing use of cough assist machine and BiPAP/C-PAP
* Recently transitioned to Palliative Care supports
*Key Responsibilities*
* Provide personal care (bathing, grooming, hygiene)
* Assist with toileting and continence care
* Support safe transfers and mobility assistance
* Meal preparation and feeding support as required
* Monitor symptoms and report changes to family/care team
* Provide compassionate emotional support
* Assist with use of respiratory equipment, including:
* Turning on/off cough assist and BiPAP/C-PAP machine
* Supporting proper placement of devices
* Cleaning and maintaining equipment
* Ensure patient comfort, dignity, and safety at all times
*Qualifications & Requirements*
* Experience in ALS and/or Palliative Care preferred
* Comfortable learning and assisting with respiratory support equipment
* Experience with transfers and limited mobility patients
* Government Issued Photo ID
* PSW Certification(s)
* Vulnerable Sector Police Background Check
* CPR Certification
